CareSource and Nomi Health Partner to Enhance Healthcare Access and Outcomes

CareSource and Nomi Health Partner to Improve Healthcare Access for Medicaid Members

CareSource, a nonprofit health plan serving over 2 million members, has partnered with Nomi Health to launch a new initiative aimed at closing healthcare gaps for Medicaid members in Ohio, Georgia, and Indiana. The collaboration seeks to enhance access to essential services and improve health outcomes for individuals facing barriers to care.

“This partnership represents a significant step forward in our mission to make every day healthier for our community,” said Misty Roberts, VP of Quality & Performance Outcomes at CareSource. “By leveraging Nomi Health’s technology and extensive provider network, we’re breaking down barriers and empowering our members to take control of their health.”

Nomi Health’s Nexus program offers comprehensive support throughout the healthcare journey, from outreach to care delivery. Key areas of focus include preventive care, such as well-child visits and postpartum care for women.

Highlights of the Nexus Program:

  • Access to a network of 900,000 providers nationwide
  • On-demand telehealth services for convenient care
  • Multi-channel engagement through SMS, email, and live agents
  • Comprehensive support for preventive care and women’s health
  • Performance tracking and analytics using electronic medical records and claims data

Sara Ratner, President of Integrated Programs at Nomi Health, emphasized the need for solutions like Nexus to address fragmented systems and limited provider access, which can lead to poor outcomes. “Our program offers a seamless, efficient care journey that meets patients where they are,” she said.

CareSource will identify members facing significant barriers, and Nomi Health will coordinate end-to-end care using telehealth, data analytics, and proactive engagement. This approach aims to reduce delays and prioritize health needs, overcoming limitations of traditional healthcare systems for more timely and effective care.

The program’s benefits extend beyond individual care, with goals to improve quality measures such as HEDIS scores and Star Ratings, while reducing unnecessary costs. This aligns with the shift towards value-based care, focusing on access and outcomes over volume.

As healthcare systems continue to struggle with access and coordination, the Nexus program by Nomi Health exemplifies how targeted interventions can address systemic issues. By streamlining care and utilizing data-driven insights, the partnership between CareSource and Nomi Health aims to set a new standard for efficient, patient-centered healthcare.
