CareSource and Nomi Health Partner to Improve Healthcare Access for Medicaid Members
CareSource, a nonprofit health plan serving over 2 million members, has partnered with Nomi Health to launch a new initiative aimed at closing healthcare gaps for Medicaid members in Ohio, Georgia, and Indiana. The collaboration seeks to enhance access to essential services and improve health outcomes for individuals facing barriers to care.

Nomi Health’s Nexus program offers comprehensive support throughout the healthcare journey, from outreach to care delivery. Key areas of focus include preventive care, such as well-child visits and postpartum care for women.

Sara Ratner, President of Integrated Programs at Nomi Health, emphasized the need for solutions like Nexus to address fragmented systems and limited provider access, which can lead to poor outcomes. “Our program offers a seamless, efficient care journey that meets patients where they are,” she said.
CareSource will identify members facing significant barriers, and Nomi Health will coordinate end-to-end care using telehealth, data analytics, and proactive engagement.
